Summary of Essential Job Duties: The Inflammatory Bowel Disease (IBD) Center Nurse Practitioner (NP) is responsible and accountable for the application of the nursing process, supervision of clinical practice, research involvement, consultative services, inter and intradepartmental liaison, education, leadership, and patient care management across the health care continuum. The Nurse Practitioner’s practice incorporates both independent decision making abilities and collaboration with the board certified/eligible physician to provide expert care to the patients. The IBD Nurse Practitioner is responsible to the medical director and the chief nursing officer (CNO) and functions as an integral part of the medical team. The Nurse Practitioner provides individualized care upholding CSHS philosophies including but not limited to care consistent with Nursing Standards of Care/Practice, the Plan for Provision of Nursing Care, the Plan for Provision of Patient Care, the Performance Improvement Plan, the Patient Education Plan, the Professional Organization Standards, Code of Conduct, and hospital and departmental policies and procedures. She/he enhances competency skills through continuing education programs. She/he demonstrates the ability to provide care to special patient population and those with diverse cultural background. She/he performs all job duties with sensitivity and attention to age and developmental milestones of the population(s) served which may include neonate, infant, pediatric, adolescent, adult, and geriatric. Essential Functions: -Conduct comprehensive, and problem focused physical, emotional, social and cultural assessments. Perform an assessment including a history taking, system focused (GI) physical examination and an assessment of a patient’s acute and/or chronic needs. -In collaboration with physician, conduct follow up clinic for established patients including patients receiving infusion treatments. -Provide acute care for established patients and communicate findings with physician. -Order laboratory and diagnostic studies to confirm diagnoses and assess the appropriateness of procedures. -Formulate an on-going plan of care in collaboration with supervising physicians while adhering to established protocols. -Coordinate patient care with multi-specialty disciplines including referring physicians, medical teaching teams, pharmacy, dieticians, nursing staffs, social services and home nursing agencies to promote optimal patient care outcomes. -Facilitate appropriate referrals such as assistance programs for medication, prior authorization for special medications and support services. -Demonstrate and model commitment to providing exceptional patient experience and the provision of patient centered care. -Participates in IBD clinical and translational research. -Identify resources and coordinates referrals for patients such as CCFA (Crohn’s and Colitis Foundation of America Inc. and patient support group meeting). -Perform other duties as determined collaboratively by the Supervising Physician that are within the scope of practice of NP’s and the health care institution’s policies. -Collaborate with internal departments and external agencies (TJC, CMS, CDPH) to ensure compliance with CSMC policy and regulatory requirements. Qualification Requirements/Preferences: Education Certifications/Licensure Experience Physical Abilities Educational Requirements: Master of Nursing Degree required Graduate of an accredited Nurse practitioner program approved by the California Board of Registered Nursing as defined in Section 1482, Nurse Practice Act, Business and Professional Code. License/Certification/Registration Requirements: Current State of California Registered Nurse and Nurse Practitioner License National Certification by respective accredited professional nursing organization such as the American Nurses Credentialing Center (ANCC), American Academy of Nurse Practitioners (AANP). Current NP Furnishing License / DEA. Current BLS/ACLS certification required. Experience: Full command (verbal and written) of the English language. Minimum of 2 (two) years acute hospital experience required Previous clinical experience in Gastroenterology, strongly preferred Physical Demands: (e.g., lifting, standing, walking) Perceptive acuity and physical ability to perform all direct and indirect patient care functions including walking, sitting, standing, bending and light lifting required.
